CYCA Crew Vaccination Declaration

As per the NSW Government Public Health Order, all crew members must be fully vaccinated to participate in CYCA races unless they have a medical exemption or are otherwise exempt. All Owners must complete the CYCA Vaccination Declaration prior their next CYCA race.

I confirm that I will ensure all crew submitted on each COVID Contact Tracing Crew List will be fully vaccinated or are otherwise legally exempt. This Declaration shall continue until the earlier of a) when I advise the CYCA differently, or b) when the Government no longer has this requirement.
