RSHYR 2021 - COVID-19 Declaration

As per SI 29.1(b) and Appendix 2 of the RSHYR 2021 Sailing Instructions, each owner must submit the following COVID-19 Declaration after 1300 hours on 23 December 2021 and before 1130 hours on 26 December 2021. The Owner must confirm all crew (unless the Tasmanian Government exempts returning Tasmanian residents) have returned a negative PCR test result and completed the Tas e-Travel documentation and received a QR Code to enter the state of Tasmania.

I confirm all crew racing on the above yacht have completed the Tas e-Travel Documentation and received a QR code to enter the state of Tasmania and have undertaken a PCR test . All crew aside from the following have received a negative result.
